In Nebraska must I get the Dr.'s approval to get a 3d ultrasound?

I am told the 3D/4D fetal ultrasound can only be done with a doctor's approval, like its a script. Is thisa correct? Please cite sources.

1. If you want to have insurance or Medicaid pay for 3D/4D ultrasound, you always must have doctor approval. If you are going to one of those places that provide it just for fun, no approval is needed. This 'fun' option is not available in all states just because there is no business offering it.
